Back and spine pain is the most common reason adults seek medical care worldwide. Regenerative medicine — including PRP injections into facet joints, the sacroiliac joint, and spinal discs — is offered by pain management and sports medicine providers as an alternative to cortisone injections or surgery.

Spinal regenerative procedures carry higher complexity and risk than peripheral joint injections. Provider selection and FDA compliance are especially important here.

Important: when to see a surgeon first

Significant nerve compression, muscle weakness, numbness radiating below the knee, or any bowel or bladder symptoms require immediate surgical evaluation. Regenerative therapy is not appropriate for these presentations.

Who is a candidate

Facet Joint Osteoarthritis

Degeneration of the small facet joints along the spine. PRP injection into facet joints is among the best-supported spinal indications, with clinical evidence comparable to cortisone at longer follow-up periods.

Sacroiliac Joint (SI) Pain

SI joint degeneration and dysfunction causing low back and buttock pain. PRP is a well-established alternative to cortisone for SI joint pain with stronger long-term outcomes in clinical studies.

Degenerative Disc Disease

Early disc degeneration without significant nerve compression. Intradiscal PRP and stem cell injection are emerging treatments. Evidence is early-stage — select providers with specific experience in spinal disc procedures.

Mild Disc Herniation

Mild herniations without severe neurological deficits or cauda equina symptoms. Significant nerve compression, weakness, or bowel/bladder involvement requires immediate surgical evaluation — regenerative therapy is not appropriate.

Facet joint and SI joint PRP has reasonable clinical support. Intradiscal procedures are earlier-stage and require more provider-specific experience.
