Rotator cuff injuries are the most common source of shoulder pain in adults over 40. The rotator cuff is a group of four muscles and their tendons that stabilize the shoulder joint — damage ranges from tendinopathy and partial tears to complete full-thickness ruptures that expose bone.

The most important thing to understand before considering regenerative therapy: not all rotator cuff injuries are the same. PRP and BMAC have a reasonable evidence base for tendinopathy and partial tears. Full-thickness tears are a different category — structural damage that typically requires a surgical consultation before regenerative therapy is appropriate.

Who is a candidate

Partial Rotator Cuff Tears

The primary indication. Partial-thickness tears — confirmed on MRI or diagnostic ultrasound — that have not responded to physical therapy or cortisone are the strongest candidates for regenerative therapy. Full-thickness tears generally require surgical evaluation first.

Rotator Cuff Tendinopathy

Chronic tendon degeneration without a discrete tear. Presents as persistent shoulder pain with overhead movement, weakness, and morning stiffness. PRP has the most evidence in this category.

Failed Conservative Treatment

Patients who completed physical therapy and cortisone injections without lasting relief. Typically 3–6 months of documented conservative care before regenerative therapy is considered appropriate.

Post-Surgical Augmentation

Some surgeons use PRP or BMAC at the time of rotator cuff repair to improve tendon-to-bone healing. This is a different clinical indication — ask specifically whether the recommendation is pre- or post-surgical.

What the evidence shows

Rotator cuff has a growing evidence base for regenerative therapy — stronger for tendinopathy than for structural tears.

Evidence level for partial tears

Multiple studies support PRP and BMAC for partial-thickness tears. Evidence is more consistent for tendinopathy than for partial tears, and weaker still for full-thickness tears, which typically require surgical repair.
